Thomas Test

The Thomas Test is designed to detect the presence of shortening or contracture of the hip flexors such as the iliopsoas muscle group.

With the patient in the supine position ask them to fully flex their uninvolved hip by grasping their knee and pulling it toward their abdomen.

Observe for any elevation of the knee on the involved side. Normally there would be no, or only small, elevation of the knee from the table.

The test is considered positive if the involved knee elevates excessively from the table, indicating hip flexor shortening or contracture on the involved side.
